I added a survey to my course so that the course participants can evaluate the course quality. But, I realized that it is not anonymous. I can see in the results csv file the answers of every participant including their names. Is there a way to make it anonymous?

Unfortunately the survey xblock will not allow you to set the survey as fully anonymous.
If this is a hard requirement, you can resort to using an external survey /web form data collection tool such as google forms.
